Job Title: In-Home Care Coordinator

Overview: We are seeking a highly motivated and compassionate In-Home Care Coordinator to join our team. As an In-Home Care Coordinator, you will play a pivotal role in ensuring that our clients receive the best quality care in the comfort of their own homes. You will work closely with our clients, their families, and our team of caregivers to create personalized care plans and coordinate all aspects of in-home care services.

Key Responsibilities:

• Conduct initial assessments of new clients to determine their care needs and create individualized care plans
• Regularly review and update care plans to ensure they meet the changing needs of our clients
• Coordinate with our team of caregivers to assign appropriate care providers for each client
• Conduct caregiver trainings and provide ongoing support and guidance to ensure high-quality care is being delivered
• Act as a liaison between clients, their family members, and healthcare professionals to ensure all care is coordinated and communicated effectively
• Assist with scheduling and coordinating appointments, transportation, and other tasks as needed
• Monitor client progress and promptly address any concerns or issues that arise
• Maintain accurate records and documentation of all client information, care plans, and services provided
• Collaborate with other members of the care team to ensure all aspects of care are being addressed
• Continuously seek ways to improve the quality of care and services provided to our clients

Qualifications:

• Bachelor's degree in Nursing, Social Work, or a related field
• Minimum of 2 years experience in care coordination or a related role
• Strong knowledge of home care services and regulations
• Excellent communication and interpersonal skills
• Ability to multitask and prioritize in a fast-paced environment
• Compassionate, empathetic, and able to build strong relationships with clients and their families
• Proficient in computer skills and able to work with various software programs
• Ability to work independently and as part of a team
• Flexible and adaptable to changing job duties and responsibilities
