Position Overview: We are seeking a highly motivated and compassionate individual to join our team as a Patient Care Coordinator. As a Patient Care Coordinator, you will be responsible for providing exceptional customer service and coordinating patient care services to ensure the best possible outcomes for our patients. You will work closely with healthcare providers, patients, and their families to ensure a seamless and positive experience throughout their healthcare journey.

Key Responsibilities:

• Serve as the main point of contact for patients and their families, providing them with support and guidance throughout their healthcare journey
• Schedule and coordinate appointments, procedures, and referrals for patients in a timely and efficient manner
• Verify insurance coverage and obtain authorizations for services
• Maintain accurate and up-to-date patient records and documentation
• Collaborate with healthcare providers to develop and implement individualized patient care plans
• Monitor and track patient progress and follow-up on any necessary interventions or changes to the care plan
• Act as a liaison between patients, healthcare providers, and other healthcare professionals to facilitate effective communication and coordination of care
• Provide education and resources to patients and their families to promote understanding and compliance with treatment plans
• Ensure compliance with all healthcare regulations and standards of practice

Qualifications:

• High School Diploma or equivalent, Bachelor's degree preferred
• Minimum of 2 years of experience in a healthcare setting, preferably in a patient care or administrative role
• Strong customer service skills and the ability to empathize with patients and their families
• Excellent communication and interpersonal skills
• Proficiency in electronic medical records and other healthcare software systems
• Detail-oriented with strong organizational and time-management skills
• Ability to work in a fast-paced and dynamic environment
• Knowledge of healthcare regulations and compliance standards
