Position: Social Worker

Overview: We are seeking a highly motivated and compassionate Social Worker to join our team. As a Social Worker, you will be responsible for providing support and services to individuals, families, and communities in need. You will work closely with clients to assess their needs, develop treatment plans, and connect them with necessary resources and support systems. The ideal candidate will have a strong understanding of social work principles and practices, excellent communication and problem-solving skills, and a passion for helping others.

Key Responsibilities:

• Conduct comprehensive assessments to identify clients' needs, strengths, and challenges
• Develop individualized treatment plans based on the assessment, in collaboration with clients and their support systems
• Provide counseling and therapy services to clients, utilizing evidence-based practices and techniques
• Connect clients with appropriate resources and support systems, such as healthcare services, housing assistance, and financial aid
• Advocate for clients' rights and needs, and assist them in navigating complex systems and processes
• Maintain accurate and up-to-date client records and documentation
• Collaborate with other healthcare professionals, social service agencies, and community organizations to ensure comprehensive care for clients
• Participate in team meetings and case conferences to discuss client progress and address any challenges or concerns
• Keep current with social work best practices and attend relevant trainings and workshops to enhance skills and knowledge

Requirements:

• Bachelor's or Master's degree in Social Work or related field
• State licensure or certification as a Social Worker
• At least 2 years of experience in a social work role, preferably in a healthcare or community-based setting
• Strong understanding of social work principles, theories, and practices
• Excellent communication, interpersonal, and problem-solving skills
• Empathetic, non-judgmental, and culturally sensitive approach
• Ability to work independently and as part of a multidisciplinary team
• Proficiency in using electronic medical records and other relevant software
• Valid driver's license and reliable transportation
