Position Overview: As a Case Manager, you will be responsible for providing comprehensive case management services to clients in need. You will work closely with individuals and families to assess their needs, develop personalized care plans, and connect them with necessary resources and services. This is a rewarding and challenging role that requires strong communication, problem-solving, and organizational skills.

Key Responsibilities:

• Conduct initial assessments to determine client needs and develop individualized care plans
• Coordinate and monitor client progress, making necessary adjustments to care plans as needed
• Connect clients with community resources and services, such as housing, healthcare, and employment assistance
• Advocate for clients and collaborate with other agencies to ensure they receive the best possible care
• Maintain accurate and up-to-date client records and documentation
• Provide crisis intervention and support to clients as needed
• Collaborate with a multidisciplinary team to ensure the best possible outcomes for clients
• Participate in ongoing training and professional development opportunities
• Adhere to all company policies and procedures, as well as ethical and legal standards

Qualifications:

• Bachelor’s degree in Social Work, Psychology, Counseling, or a related field
• Minimum of 2 years of experience in case management or a related field
• Knowledge of community resources and services
• Strong communication, problem-solving, and organizational skills
• Ability to work independently and collaboratively as part of a team
• Compassionate and empathetic attitude towards clients
• Proficient in Microsoft Office and database management
• Valid driver’s license and reliable transportation
