Position: Case Manager
Job Description: We are looking for a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will be responsible for providing support and guidance to individuals who are in need of assistance with their daily living activities. You will work closely with clients to develop and implement individualized care plans to ensure their physical, mental, and emotional well-being.
Key Responsibilities:
- Conduct initial assessments to determine the needs and goals of clients
- Develop and implement individualized care plans based on clients' needs and goals
- Provide ongoing support and guidance to clients to help them achieve their goals
- Collaborate with healthcare professionals and other service providers to ensure comprehensive care for clients
- Monitor and evaluate clients' progress and make necessary adjustments to care plans as needed
- Educate clients on available resources and assist in connecting them with community services
- Advocate for clients and ensure their rights and needs are being met
- Maintain accurate and up-to-date documentation of all client interactions and progress
- Participate in case conferences and team meetings to discuss clients' care plans and progress
- Adhere to all ethical and legal standards while working with clients
- Provide crisis intervention and support to clients as needed
- Maintain confidentiality and privacy of all client information
Qualifications:
- Bachelor's degree in Social Work, Psychology, or related field
- Minimum of 2 years of experience in case management or related field
- Knowledge of social service resources and community services
- Excellent communication and interpersonal skills
- Strong organizational and time-management skills
- Ability to work independently and as part of a team
- Compassionate and empathetic nature
- Proficient in Microsoft Office and other computer applications
- Ability to handle sensitive and confidential information with professionalism and discretion
