About the Role: We are currently seeking a dedicated Mental Health Case Manager to join our team. In this role, you will be responsible for providing case management services to individuals with mental health disorders. You will work closely with clients to develop personalized treatment plans and provide ongoing support to promote their overall well-being.

Key Responsibilities:

• Conduct comprehensive assessments of clients' mental health needs
• Develop individualized treatment plans in collaboration with clients, their families, and other healthcare professionals
• Provide ongoing case management services, including coordinating and monitoring services, referrals, and follow-up care
• Assist clients in accessing community resources and support services
• Develop and maintain relationships with external agencies and service providers to ensure the best possible care for clients
• Advocate for clients' rights and needs within the healthcare system
• Monitor and document clients' progress and make necessary adjustments to treatment plans
• Provide crisis intervention and support as needed
• Collaborate with a multidisciplinary team to ensure integrated and holistic care for clients
• Stay updated with current mental health trends, research, and best practices to continuously improve service delivery

Qualifications:

• Bachelor's or Master's degree in Social Work, Psychology, or a related field
• Minimum of 5 years of experience in case management, preferably in a mental health setting
• Strong knowledge of mental health disorders, treatment options, and community resources
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Strong organizational and time-management skills
• Ability to handle crisis situations with sensitivity and professionalism
• Proficiency in electronic medical records and case management software
• Valid driver's license and access to a reliable vehicle
