About the Role: We are seeking a highly motivated and compassionate Social Work Liaison to join our team at AssistCare Network. As a Social Work Liaison, you will be responsible for providing support and assistance to individuals and families who are facing difficult situations. You will work closely with social workers and other healthcare professionals to coordinate and facilitate services for our clients.

Key Responsibilities:

• Collaborate with social workers and other healthcare professionals to develop and implement individualized care plans for clients
• Conduct assessments and gather information to determine the needs of clients
• Provide counseling and support to clients and their families
• Connect clients with community resources and services to meet their needs
• Advocate for clients and assist them in navigating the healthcare and social service systems
• Maintain accurate and detailed records of client interactions and progress
• Participate in case conferences and team meetings to discuss client care and progress
• Collaborate with other departments within the organization to ensure seamless care for clients
• Stay up-to-date with current social work practices and regulations to provide the best possible support to clients
• Develop and maintain positive relationships with clients, their families, and other stakeholders
• Adhere to ethical standards and maintain confidentiality of client information at all times

Qualifications and Skills:

• Bachelor's or Master's degree in Social Work
• Valid social work license in CA
• Minimum of 5 years of experience in social work
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Strong organizational and time-management skills
• Empathetic and non-judgmental approach towards clients
• Knowledge of community resources and services
• Familiarity with electronic medical records systems
• Ability to handle sensitive and confidential information with discretion
