Job Title: Social Worker for SupportUnity Services

Position Overview: SupportUnity Services, a leading non-profit organization, is seeking a highly motivated and compassionate Social Worker to join our team. As a Social Worker for SupportUnity Services, you will play a crucial role in providing support and services to individuals and families in need. You will work closely with a diverse team to assess the needs of clients, develop personalized care plans, and connect them with necessary resources to improve their overall well-being.

Key Responsibilities:

• Conduct comprehensive assessments of clients' mental health, physical health, and social needs
• Develop personalized care plans in collaboration with clients, their families, and other healthcare professionals
• Provide counseling and support services to clients, helping them cope with their challenges and develop necessary skills for self-sufficiency
• Connect clients with community resources and services such as housing, healthcare, employment, and education
• Advocate for clients and their families to ensure their needs are met and their rights are protected
• Monitor and evaluate clients' progress, making necessary adjustments to their care plans as needed
• Maintain accurate and up-to-date documentation of all client interactions and services provided
• Collaborate with other professionals and agencies involved in clients' care to ensure coordinated and comprehensive services
• Stay current with social work best practices and attend professional development trainings as required

Qualifications:

• Bachelor's or Master's degree in Social Work from an accredited university
• Current state social work license or ability to obtain one within 90 days of employment
• Minimum of 2 years of experience in social work or related field
• Knowledge of social work principles, methods, and ethics
• Ability to assess and analyze complex situations and develop effective care plans
• Excellent communication, interpersonal, and case management skills
• Proficient in using technology and electronic health records
• Strong organizational and time-management skills
• Ability to work independently and as part of a team in a fast-paced environment
• Compassionate, non-judgmental, and culturally sensitive approach to working with diverse populations

Working Conditions:

• This is a Full-time position, with some evenings and weekends as needed
• Ability to travel within the local community to meet with clients and attend meetings
• Physical demands include sitting, standing, and walking for extended periods of time, and lifting up to 25 pounds
• Must have a valid driver's license and reliable transportation
