Position: Case Manager for Individual & Family Services
Job Description: We are seeking a highly motivated and compassionate individual to join our team as a Case Manager for Individual & Family Services. This role is crucial in providing support and guidance to individuals and families in need, and requires a strong understanding of social work principles and practices.
Key Responsibilities:
- Conduct comprehensive assessments of clients' needs and develop individualized service plans
- Provide case management services to individuals and families, including coordination of resources and referrals
- Monitor clients' progress and make necessary adjustments to service plans
- Collaborate with other professionals and agencies to ensure holistic and effective care for clients
- Advocate for clients' rights and needs within the community
- Maintain accurate and detailed case notes and client records
- Facilitate counseling and support groups for clients as needed
- Participate in team meetings and case conferences to discuss clients' progress and develop strategies for improvement
- Keep up-to-date with current social work practices and participate in relevant training and professional development opportunities
Key Requirements:
- Bachelor's or Master's degree in Social Work or related field
- Minimum of 2 years of experience in case management or social work
- Knowledge of social work principles, theories, and practices
- Ability to work independently, as well as part of a multidisciplinary team
- Excellent communication and interpersonal skills
- Empathetic and non-judgmental approach to working with clients
- Strong organizational and time-management skills
- Proficiency in computer applications and electronic record keeping
- Valid driver's license and reliable transportation
